I don't remember using a flash on this photo, but if I did, it was reflected off the back of an ipod so as to bounce off the ceiling. I've only got an on-board flash on my Nikon D80, but with enough diffusion and bouncing, you can get some pretty good results.
The photo I just posted today used a built-in flash bounced off a vanity mirror to the ceiling in a VERY dark room. http://levibethune.com/?p=600
The above photo looks like it was shot with available light. I know I used my 15mm fisheye at f/2.8, but I'd have to dig up the raw and look at the exif data to be sure.

I know you were using a spoon to diffuse the flash some before this was taken - maybe here too?
